Evolent Health is actively recruiting for a Health Educator to join a team of clinical colleagues working collectively to improve the health of a defined population. This position will be focused on moderate risk/at risk patients living with chronic disease(s). The Health Educator will assist our patients and caregivers in finding new ways to inspire healthier behaviors and build the self-management skills of our patients and care givers. The individual taking this role will manage their caseload through telephonic and electronic communications and coordination.

Responsibilities

Conducts initial outreach calls to encourage patients and care givers to participate in the program
Contacts and performs initial interviews/screenings with eligible patients.

Provides self-management and care coordination support for patients with one of the following conditions: Asthma, Chronic Obstructive Pulmonary Disease, Coronary Artery Disease and Diabetes.

> Assists the patient and care giver to comprehend their illness, make life style changes, adhere to medication regimen and work with their provider to develop a symptom response plan

> Creates a personalized action plan that will be reviewed with an RN Care Advisor. The action plan will include short and long term goals prioritized by the patient, specific activities/interventions to achieve goals. Identify and address barriers that are preventing the patient from meeting their goals.

> Conducts regularly scheduled, generally every 3 weeks, self-management coaching, education calls about the patient’s condition and role in taking care of themselves; this includes care coordination with patients, care givers, physicians and other community and health plan based resources to ensure patients are receiving the right care in the right place at the right time;

> Arranges referrals to Evolent extended care team members including registered nurses, social workers and pharmacist, as well as, network/delivery system providers and services;

> Documents their activities in the Identifi Care system, including action plan and activities conducted

> Educates the patient and/or care giver about symptom response plans

> Arranges transportation

> Facilitates gaps in care closure by educating patients about preventive monitoring/screening and working with physician practices to schedule diagnostic testing

> Adheres to documentation standards and policies and procedures; including frequency of outreach

> Comprehends and promotes Evolent and Client goals, including program metrics, improvements in quality, cost of care, and member experience for the population that is managed.

> Identifies opportunities for improvement (at individual, client and program levels) and actively partners with management and the broader Evolent clinical team to achieve and/or improve results

Qualifications:
Required

Two years of experience working with commercial, Medicare and/or Medicaid chronic populations to provide health education, self-management, wellness, care coordination support and lifestyle related topics.

Respiratory Therapist, Registered Dietitian, RN or LPN with B.S. in Health Education, Health promotion, Community Health Education or equivalent education.

Experience working in a health plan setting or doing predominantly telephonic health education and coaching is not required but it is highly preferred

Ability to unite with people with diverse needs and background through the telephone

>Health Coach Certification completed prior to or within the first two years of employment at Evolent

On-Call Health Educators support the HealthFitness mission to improve the health and well-being of the people we serve, by delivering in-person and web-based health education seminars and group training classes on a variety of wellness subjects.

Health Educators provide engaging, practical, science-based seminars in a group setting to help raise awareness of healthy choices and reduce health risks among employees. HealthFitness educational seminars are one-hour seminars in diabetes, back health, stress, heart health, blood pressure, weight, women’s health, sleep, nutrition, physical activity and financial wellbeing.

Health Educators also deliver group wellness classes to support individuals in transforming the way they approach healthy behaviors and use a coaching approach to help participants lose weight, quit smoking, eat healthier, manage stress, and increase exercise. The HealthFitness group wellness classes are results-oriented group training and motivational programs that occur weekly over the period of one to several months.

Health Educators work from home and deliver seminars and classes at various regional client locations. Travel may be requested overnight and on weekends. This position is “on-call”, hours are not guaranteed, and full time openings are not available. Associates will receive notifications for local and non-local seminar opportunities and may sign up based on their availability.

Job Description

Reporting to CCC’s Medical Director, the Population Health Specialist at CCC will drive forward initiatives to improve population health management and the care of its members with chronic illnesses at CCA’s flagship specialty clinical practice serving people with disabilities and elders with complex medical needs. This position is responsible for implementing processes that ensure the delivery of evidence based population health interventions. This will entail tracking these members and then engaging with CCC providers on interventions. This position will ensure that CCC members with diagnoses that require periodic maintenance get all appropriate care. It is an ideal job for someone with an interest in health care administration. In addition, this position is responsible for tracking and intervening upon CCC members who have medical conditions that place them at high risk for ED visits and inpatient admissions.

Essential Duties & Responsibilities

  • Is available Monday – Friday 8 a.m. – 5 p.m. to provide operational and administrative support for all CCC clinicians across all 4 sites
    • Attends clinical team meetings at home site
  • Track, identify, and intervene upon all CCC members who are deficient in population health management in a recurring manner
    • Establish and use a shared tracking system (i.e. Excel sheet in a shared CCC drive) to identify which members require population health interventions
    • Organize existing clinic staff, including RNs, LPNs, and paneled clinicians, to intervene on these deficiencies
    • Ensure that this process occurs annually in an automatic, recurring fashion
  • Track members who have diagnoses that require periodic maintenance (such as cirrhosis, diabetes, hypertension, schizophrenia, etc.)
    • Maintain a shared tracking system comprised of members who have these diagnoses
    • Work with existing clinic staff, including RNs, LPNs, pharmacists, and paneled clinicians (APC and physician) to provide periodic maintenance to these members
  • Track members who have high risk diagnoses and then coordinate timely and recurring care for these diagnoses
    • Maintain a shared tracking system comprised of members who have these diagnoses
    • Work with existing clinic staff, including RNs, LPNs, pharmacists, and paneled clinicians (APC and physician) to care for these members in a timely and recurring fashion
  • Arrange education sessions to clinicians as needed regarding population health management interventions, i.e. updates on colorectal cancer screening
  • Collaborate with quality team at CCA to ensure that CCC’s population health management is consistent with CCA’s vision
